These enhancements go beyond traditional Medicare, giving you more flexibility and support when you need it most.

Here’s how you may benefit:

    • Skilled Nursing Facility (SNF) Waiver – No 3-Day Hospital Stay Required
      Normally, Medicare requires a three-day hospital stay before covering a skilled nursing facility admission. With this waiver, you may be eligible for SNF care without a hospital stay if your care team decides it’s right for you—helping you get the care you need sooner.

    • Concurrent Care While in Hospice
      If you choose hospice care, you don’t have to give up treatments aimed at improving your condition. You may be eligible to receive both hospice services and curative treatments at the same time, ensuring comfort and hope can go hand in hand.

    • Home Health Without the “Homebound” Requirement
      Traditional Medicare requires patients to be considered “homebound” to receive home health services. With this waiver, you may still be eligible for in-home care even if you are not homebound—making it easier to recover safely in the comfort of your own space.

Your Rights and Choices

  • You Always Choose Your Doctor: Even if your provider is part of an ACO, you can still see any doctor or specialist who accepts Medicare.

  • Privacy and Data Sharing: You can opt out of sharing your data with the ACO at any time while still receiving care.

  • Annual Notification: Each year, Medicare will let you know if your doctor is participating in an ACO. Nothing about your Medicare benefits changes.
